Members of the CareSpark Clinical workgroup identified five priority health issues to be targeted through this effort:

• Diabetes
• Hypertension and Stroke
• Cardiovascular Disease
• Asthma and Lung Disease
• Cancer

In our Central Appalachian region, these are the health issues that have the greatest impact on health and overall quality of life, as well as high costs for patients with poor outcomes.
